LNA  Licensed Nursing Assistant
CNA  Certified Nurse Assistant

Position Summary
The Licensed Nurse Assistant (LNA) or Certified Nurse Assistant (CNA) is responsible for performing direct patient care as assigned by a Registered or Licensed Nurse to meet identified patient outcomes. This position typically reports to the Charge Nurse or the Director of Nursing.

Main Responsibilities
Performs patient care and related activities as directed by the nurse
Administers patient’s personal hygiene regimen, including but not limited to bathing, dressing, skin care and oral hygiene
Positions patients as indicated for maintenance of correct body alignment
Answers call lights/bells and makes rounds
Makes beds and change linens; Transfers patients to and from bed or chair or other locations within the facility
Assists patients to and from the bathroom, assist with any toileting needs
Performs specific interventions to meet elimination needs by providing catheter care and administering enemas
Provides interventions to meet patients nutritional needs
Records and reports I&O and calorie count after every meal
Completes the following according to unit routine: vital signs, height/weight, intake/outtake, urine/stool specimen, stool guaiac testing, simple dressings, bowel and bladder training
Demonstrates continued competence in caring for individuals based upon age-specific needs utilizing a developmental perspective
Displays an understanding of each patient’s cognitive, physical, emotional and chronological maturity in assessments, treatment and care
Modifies approaches appropriately to the patient based upon patient age
Provides appropriate interventions related to safety issues
Records activities of daily living on flow sheets
Observes residents and notes physical condition, attitude, reactions and appetite
Assists with admitting, transferring, and discharging residents
Assists nurse in giving treatments and examining residents
Assists with restorative and rehab care with proper instruction and supervision

Communication
Communicates patient information to assure confidentiality and continuity of care
Interacts with all patients, families, visitors and fellow employees in a mature, responsible manner to ensure positive and professional environment

Physical Requirements
Visual acuity and accurate color vision
Ability to lift and transfer patients in excess of 100 pounds which may require pushing, pulling and essentially utilizing a full range of body movement
Ability to stand, walk, stoop, kneel, crouch and crawl

Equipment Used
Stethoscope, blood pressure cuff, scale, tray, carts, hoyer lift, whirlpool tub, wheelchairs, walkers, canes, charts, linens, towels, standard precautions (gloves, goggles, mask, aprons), thermometers, urinals, bed pans, commodes, and personal hygiene supplies
